Σας καλωσορίζουμε στην 4η σειρά σεμινάριου παρακολούθησης ζωντανής εγχείρησης ώμου, για προχωρημένους στο Annecy της Γαλλίας.
Από την πρώτη διοργάνωση το 2005, είναι σε παγκόσμια κλίμακα η πρώτη και μεγαλύτερη συνάντηση τέτοιου είδους, που αποτελείται αποκλειστκά από «ζωντανές» εγχειρήσεις. Με μεγάλη προσπάθεια συνεχίζουμε αυτό το σεμινάριο, με το ενδιαφέρον να είναι τεράστιο παγκοσμίως. Μέσα στο 2009 είχαμε περισσότερους από1000 επισκέπτες, εκ των οποίων περισσότερους από 800 ορθοπεδικούς από 40 χώρες και από τις 5 ηπείρους. Σε αυτό το σημείο θα θέλαμε να ευχαριστήσουμε την υπέροχη διεθνή μας εκπαιδευτική ομάδα, η οποία συμπεριλαμβάνει κάποιους από τους γνωστότερους χειρούργους ώμων στον κόσμο. Ο χρόνος και οι γνώσεις που προσφέρουν αυτοί μας οι συνάδελφοι, καθορίζουν και την επιτυχία του σεμιναρίου. Κατ’ αυτόν τον τρόπο, με την μετάδοση και μεταβίβαση των γνώσεων επί του θέματος, ελπίζουμε να διευκολύνουμε τον κλάδο να αποκτήσει μια γενική εικόνα σχετικά με τις νεώτερες και σύγχρονες τεχνικές εγχείρησης ώμων.
Η εγχείρηση ώμου είναι σχετικά ένας καινούριος τομέας με πολύ γρήγορη εξέλιξη. Τα δύο τελευταία χρόνια η αύξηση των γνώσεων, κυρίως στον τομέα της εξωαρθρικής ενδοσκόπησης της ανατομίας του ώμου, βοήθησε τόσο στην καλύτερη κατανόηση της παθολογίας του όσο και των θεραπευτικών δυνατοτήτων αυτού. Η έρευνα και η γνώση αυξάνονται με ταχείς ρυθμούς δυσκολεύοντας τους ορθοπεδικούς να έχουν επαφή με τα δρώμενα.
Με παρακολουθήσεις εγχειρήσεων ζωντανά, καθίσταται δυνατόν η ανάλυση των καινούριων και σύγχρονων μεθόδων των εγχειρήσεων του ώμου που είναι και το κεντρικό μέρος του σεμιναρίου. Καθ` όλη την διάρκεια του 4ήμερου σεμιναρίου θα διεξαχθούν 33 ζωντανές εγχειρήσεις από τους διεθνείς μας ομιλητές. Όπως και το 2009 θα πραγματοποιηθούν εγχειρήσεις που σχετίζονται με ανατομικά προβλήματα και όχι παθολογικά. Επιλέχθηκαν εγχειρήσεις όπου παθολογικά προβλήματα είναι συνδεδεμένα με ανατομικά.

Η πρώτη μέρα (Πέμπτη 2 Ιουνίου) είναι αφιερωμένη στην οπίσθια άνω ωμική περιοχή: Αποκατάσταση άνω χείλους, οπίσθιου χείλους της ωμογλήνης καθώς και αποσπαθέντος οστικού τεμαχίου της ωμογλήνης, αποκατάσταση υπερακάνθιου και υπακάνθιου μυός, αποσυμπίεση υπερπλάτιου νεύρου, σταθεροποίηση ακρωμιοκλειδικής άρθρωσης.

Την δεύτερη μέρα (Παρασκευή 3 Ιουνίου) θα παρουσιαστούν παθολογικά προβλήματα της πρόσθιας ωμικής περιοχής όπως αποκατάσταση πρόσθιου χείλους της ωμογλήνης, αποκατάσταση υποπλάτιου μυός καθώς και αποσπασθέντος οστικού τεμαχίου της ωμογλήνης.

Την Τρίτη μέρα (Σάββατο 4 Ιουνίου) θα είναι αφιερωμένη στην αρθροπλαστική ώμου.

Την τέταρτη μέρα (Κυριακή 5 Ιουνιού) θα επικεντρωθούμε στην κάτω ωμική περιοχή με το βραχιόνιο πλέγμα και πιο συγκεκριμένα την αποσυμπίεση του μασχαλιαίου νεύρου κατά την διέλευση του από τον πλατύ ραχιαίο μυ.

Στο τέλος κάθε μέρας θα ακολουθεί συζήτηση με τους χειρούργους ορθοπεδικούς.
Για κάθε περίπτωση θα δείχνονται όλες οι πτυχές που αφορούν τις χειρουργικές μεθόδους. Φέτος θα δοθεί βαρύτητα στην παρουσίαση ανοιχτών και αρθροσκοπικών επεμβάσεων της ανάλογης ανατομίας του ώμου, στους τρόπους χειρισμού οστικής απώλειας στα πλαίσια μιας αστάθειας, καθώς επίσης θα γίνει μια εμβάθυνση σε οτι αφορά την λεπτομέρεια και τις επιλογές για την σωστή αποκατάστασητου πετάλου των στροφέων. Στόχος στην ημερίδα της χειρουργικής αρθροπλαστικής είναι να εστιάσει στην διαχείρηση των μαλακών ιστών και να γίνει χειρουργική αποκάλυψη της άρθρωσης αντί των συγκεκριμένων τύπων αρθροπλαστικής που χρησιμοποιούνται. Πάνω σε αυτούς θα γίνει συστηματική ανάλυση κατά τη διάρκεια των εργαστηρίων.
Όπως και στο τελευταίο σεμινάριο το 2009 θα χρησιμοποιηθούν εικονικές παραστάσεις του ώμου (ζωντανές τρισδιάστατες), για την καλύτερη παρουσίαση της ανατομίας και παθολογίας διευκολύνοντας κατ` αυτό τον τρόπο την συνήθη δισδιάστατη εικόνα. Στόχος μας είναι κατά τη διάρκεια του σεμιναρίου το 2011, μέσα από 33 ζωντανές εγχειρήσεις, να δείξουμε τις πιο σύγχρονες τεχνικές σε πραγματικό χρόνο, και τον τρόπο με τον οποίο οι καλύτεροι και πιο έμπειροι ορθοπεδικοί της γης χειρίζονται αυτές τις καταστάσεις. Παρά τις όποιες δυσκολίες που ενδεχομένως παρουσιαστούνε κατά τη διάρκεια του σεμιναρίου, όλες οι περιπτώσεις θα ξεκινήσουν και τελειώσουν σύμφωνα με το πρόγραμμα. Μία από τις πιο ενδιαφέρουσες και συνάμα εκπαιδευτική πτυχή του σεμιναρίου, είναι η ζωντανή παρακολούθηση του τρόπου αντιμετώπισης και επίλυσης διάφορων προβλημάτων που θα παρουσιαστούν, από τους διασημότερους και πιο έμπειρους χειρούργους του ώμου. Οι συντονιστές θα είναι η σύνδεση μεταξύ του χειρούργου ορθοπεδικού και του ακροατηρίου. Ελπίζουμε οι συμμετέχοντες να έχουν ενεργητικό ρόλο στην όλη διαδικασία, για να στεφθεί ξανά με επιτυχία το σεμινάριο.

Benvenuti alla quarta edizione di « Annecy Live Surgery Shoulder Advanced Course ».

Fin dall’evento inaugurale nel 2005, questo è stato il primo e il più grande meeting al mondo basato esclusivamente sulla chirurgia « live ».
Abbiamo continuato a organizzare questo corso biennale che ha suscitato un enorme interesse mondiale. Nel 2009 più di 1000 persone (800 chirurghi provenienti da quarantotto paesi dai 5 continenti) si sono riunite a Annecy per l’evento.
Ci sembra doveroso esprimere la nostra più profonda gratitudine all’eccezionale « faculty » internazionale, formata dai migliori chirurghi della spalla di tutto il mondo, per offrirci il loro tempo e la loro esperienza che rende questo corso un successo.

Speriamo che gli esperti della « faculty », nel ruolo di operatori e di moderatori, interagendo e scambiando le loro opinioni con trasparenza, riusciranno a fornire a ciascuno di Voi un’appropriata conoscenza delle più avanzate e attuali tecniche chirurgiche disponibili oggigiorno in chirurgia della spalla.
La chirurgia della spalla progredisce a un ritmo straordinario. Di conseguenza, negli ultimi due anni, c’è stato un notevole incremento nella nostra esperienza circa l’anatomia artroscopica della spalla « Out-of-the-Box », delle sue patologie e delle possibilità terapeutiche disponibili.
I chirurghi della spalla sono oggi in grado di analizzare meglio le patologie articolari ed extra-articolari ed hanno sviluppato metodi tecnicamente avanzati per la gestione delle stesse. La scienza avanza rapidamente ed è sempre più impegnativo per il chirurgo mantenere il passo in quest’ambiente in continuo divenire!

Come in ogni nostro corso in « live surgery », Vi proporremo lo stato dell’arte delle più avanzate idee e delle più aggiornate tecniche chirurgiche in tempo reale e continueremo a organizzare i nostri corsi con quest’obiettivo fisso.
Durante i 4 giorni dell’evento la nostra « shoulder faculty » internazionale eseguirà 38 interventi in diretta. Come nel 2009 rispetteremo il principio esclusivo della « live surgery » e il programma sarà basato piuttosto sull’anatomia regionale che sulla patologia. Abbiamo selezionato delle procedure chirurgiche per far fronte alle numerose patologie rispetto a ciascuna specifica regione anatomica.

Il primo giorno (Giovedì 2 Giugno) sarà dedicato alla diagnosi artroscopica e al trattamento della patologia della spalla postero-superiore, come la riparazione delle SLAP, del « labrum » posteriore, procedure ossee posteriori, riparazione del Sopra e del Sotto Spinato, accesso e neurolisi del nervo Soprascapolare, stabilizzazione dell’articolazione AC.

Il secondo giorno (Venerdì 3 Giugno) sarà dedicato interamente all’artroplastica.

Il terzo giorno (Sabato 4 Giugno) sarà orientato alla gestione della regione anteriore della spalla, alla riparazione del « labrum » anteriore, del sottoscapolare così come le procedure ossee anteriori e di Latarjet.

Durante il quarto giorno (Domenica 5 Giugno), infine, metteremo a fuoco le patologie della spalla inferiore, come la liberazione del plesso brachiale e, più specificatamente, la neurolisi del nervo ascellare e la trasposizione del Gran Dorsale. Alla fine di ciascuna giornata tutti i membri della faculty si ritroveranno insieme per un dibattito finale.

Tutti gli aspetti della gestione chirurgica delle patologie affrontate in diretta saranno ridiscussi.

E ancora, quest’anno, si dedicherà maggiore attenzione all’anatomia della spalla « open » e artroscopica, alla gestione delle perdite ossee nell’instabilità e ad alcuni dettagli e scelte tecniche inerenti alla riparazione della cuffia in « double-row ». L’obiettivo per la chirurgia protesica sarà di mettere a fuoco la manipolazione dei tessuti molli e l’accesso chirurgico all’articolazione piuttosto che il tipo d’impianto utilizzato. Le protesi saranno adeguatamente presentate durante i workshop.

Come nel 2009, un’animazione permanente « live virtual 3D shoulder » farà da supporto nella visualizzazione e per la discussione dell’anatomia e della patologia oggetto del trattamento artroscopico che è, per definizione, limitato ad una rappresentazione bidimensionale.

Lo scopo del meeting 2011 è quello di mostrare, attraverso 38 interventi live, le tecniche più attuali e di imparare, in tempo reale, come i migliori chirurghi del mondo e i moderatori fanno fronte alle diverse situazioni e alle eventuali difficoltà. Nonostante le difficoltà intercorrenti che potranno presentarsi durante una chirurgia live, inizieremo e termineremo i corsi secondo la tabella di marcia stabilita.
L’opportunità di osservare come reagiscono i migliori chirurghi del mondo di fronte alle difficoltà occasionali è certamente uno dei punti più interessanti e istruttivi di questo meeting.
I moderatori faranno da ponte costante tra l’auditorio e il chirurgo operatore. Ci auguriamo che anche Voi facciate parte attiva della discussione e del processo di apprendimento dinamico per rendere questo evento, ancora una volta, un successo!

Bienvenue à la 4ème Edition du congrès « Annecy live surgery shoulder advanced course 2011 » !

Depuis l’évènement inaugural de 2005, qui a été le premier congrès mondial, uniquement basé sur la chirurgie en direct. Devant l’énorme intérêt qu’il a suscité nous avons perpétué l’organisation de ce cours biannuel.

En 2009, plus de 1000 personnes, dont plus de 800 chirurgiens venus des 40 pays et des 5 continents, ont été rassemblées, à Annecy.

Nous renouvelons nos remerciements à la formidable Faculté Internationale des meilleurs chirurgiens de l’Epaule, du monde entier, pour nous offrir leur temps et leur expertise et faire de ce cours, un véritable succès ! Nous espérons que cette Faculté Experte, Chirurgiens et Modérateurs, interagissant et partageant d’une manière transparente, vous procurera, une large connaissance des techniques de pointe de la chirurgie de l’Epaule d’aujourd’hui !

La chirurgie de l’Epaule avance prodigieusement. Ainsi, durant les deux dernières années, il y a eu un accroissement spectaculaire de notre connaissance du « Out of the Box »; l’anatomie endoscopique extra-articulaire de l’Epaule ainsi que les pathologies et les options de traitement validé disponibles. Les chirurgiens de l’Epaule peuvent, maintenant, mieux analyser et appréhender les pathologies et ont développé des méthodes techniquement plus avancées. La science progresse très rapidement et cela devient de plus en plus difficile pour le chirurgien, de garder le rythme, dans un environnement aussi rapidement changeant !

Comme cours de chirurgie en direct, nous offrons le plus possible l’échange, en temps réel, avec le meilleur inter-partage d’idées chirurgicales et de techniques de pointe, et nous continuons à organiser ce cours dans cet esprit. Pendant ces 4 jours d’évènement, notre Faculté Internationale de l’Epaule réalisera 38 chirurgies en direct. Comme en 2009, nous adhérerons au concept de chirurgie exclusivement en direct et le programme sera basé sur l’anatomie régionale et non pas sur la pathologie. Nous avons sélectionné des procédures chirurgicales à traiter avec une variété de pathologies différentes selon une région spécifique de l’Epaule.

Le 1^er jour (jeudi 2 juin) sera consacré au diagnostic et au traitement arthroscopique de la pathologie de l’épaule postéro-supérieure, incluant : réparation de SLAP, réparation du Labrum Postérieur et procédures des parties osseuses postérieures, réparation du sus & sous-épineux, abord et décompression du nerf Supra-scapulaire, stabilisation de l’articulation acromio-claviculaire.

Le 2^ème jour (vendredi 3 juin) sera entièrement consacré à l’Arthroplastie.

Le 3^ème jour (samedi 4 juin) sera dédié au traitement Arthroscopique de la région Antérieure de l’Epaule, incluant la réparation du labrum antérieur, de la réparation du tendon sous-scapulaire, ainsi que le bloc osseux antérieur et les procédures de la butée (Latarjet).

Le 4^ème jour (dimanche 5 juin), sera centré sur la région Inférieure de l’Epaule, y compris, le Plexus Brachial et, plus spécifiquement, la libération du Nerf Axillaire, ainsi que le transfert du grand dorsal (Latissimus Dorsi).

À la fin de chaque journée, tous les membres de la Faculté se réuniront pour  la discussion finale.

Tous les aspects de techniques chirurgicales pour ces pathologies, vous seront démontrés. Cependant, cette année,  plus de temps sera dédié à l’anatomie de l’Epaule  sous arthroscopie et à ciel ouvert ; l’indication de perte osseuse dans l’instabilité de l’Epaule, ainsi que sur les détails pertinents et options pour la double rangée de fixation de la Coiffe des Rotateurs. L’objectif pour le chirurgien qui réalisera sa chirurgie d’arthroplastie devra se concentrer sur le maniement  des tissus mous de l’articulation plutôt que sur les types spécifiques d’arthroplastie utilisée. Les prothèses seront entièrement analysées pendant les sessions d’atelier.

Comme en 2009, une Epaule virtuelle en 3D, permanente, vous aidera dans la visualisation aussi bien  pendant les discussions de l’anatomie que les pathologies traitées de chaque arthroscopie. Le but de ce congrès 2011 est de montrer, à travers ses 38 chirurgies en direct, les techniques de pointe et d’apprendre, en temps réel, comment les meilleurs chirurgiens et chirurgiens modérateurs font face à ces différentes situations et aux difficultés rencontrées. C’est de la chirurgie d’Epaule en direct, transparente et de pointe. Malgré les difficultés occasionnelles qui peuvent survenir pendant leur chirurgie, nous commencerons et finirons à l’heure. L’opportunité d’observer les meilleurs chirurgiens mondiaux réagissant et évaluant ces difficultés, est certainement un des points culminants, les plus intéressants et instructifs, de ce congrès. Les modérateurs seront la liaison permanente entre l’audience et le chirurgien opérant. Chciałbym bardzo serdecznie zaprosić na 4. Kongres Chirurgii Barku „Live” w Annecy

Od pierwszego inauguracyjnego zjazdu w Annecy w 2005 roku, jest to największy światowy kongres naukowy, polegający na bezpośredniej transmisji z sali operacyjnej „na żywo”. Kongres powtarzany jest co 2 lata i od początku budzi ogromne zainteresowanie. W 2009 roku wzięło w nim udział ponad 1000 osób, w tym ponad 800 chirurgów z 48 państw (5 kontynentów).

Jesteśmy niezmiernie wdzięczni wszystkim najlepszym chirurgom barku z całego świata – członkom komitetu naukowego, za ich poświęcony czas i doświadczenie, które przyczyniły się do sukcesu naszej konferencji. To właśnie ci chirurdzy i moderatorzy zapewniają Państwu możliwość poznania najnowszej wiedzy i technik operacyjnych w chirurgii barku.

Chirurgia barku rozwija się obecnie w nadzwyczajnym tempie. Od 2 lat nastąpił ogromny postęp w naszej wiedzy o chirurgii endoskopowej „poza stawem” – anatomii, patologii i możliwościach leczenia. Chirurdzy barku mogą w tej chwili lepiej diagnozować patologie oraz wykorzystywać nowe umiejętności techniczne, które pozwalają na ich leczenie. Przy tak szybkim postępie, niezwykle trudnym wyzwaniem dla chirurga jest utrzymywanie „na czasie” swojej wiedzy i umiejetności.

W związku z faktem, że podstawą naszego kongresu jest chirurgia „na żywo”, każdy z obserwatorów będzie miał okazję zdobyć wiedzę teoretyczną, jak i zobaczyć najnowsze i najbardziej zaawansowane techniki chirurgiczne. Podczas 4. dniowego zjazdu, członkowie komitetu naukowego wykonają 38 operacje barku. Podobnie jak w 2009 roku, podział operacji będzie opierał się na anatomii barku – operacje będą dotyczyły patologii określonych okolic anatomicznych barku.

Pierwszy dzień (czwartek, 2 czerwca) będzie poświęcony artroskopowej diagnostyce i leczeniu tylno-górnej okolicy barku: uszkodzenia typu SLAP, rekonstrukcja tylnego obrąbka, tylne stabilizacje kostne, uszkodzenia ścięgna mięśnia nadgrzebieniowego i podgrzebieniowego, uwolnienie nerwu nadłopatkowego oraz stabilizacja stawu barkowo-obojczykowego.

Dzień drugi (piątek, 3 czerwca) będzie całkowicie poświęcony artroplastyce barku.

Dzień trzeci (sobota, 4 czerwca) będzie dotyczył artroskopii przedniej okolicy barku: naprawa uszkodzeń przedniego obrąbka, rekonstrukcja ścięgna mięśnia podłopatkowego, a także przednie stabilizacje kostne.

Czwarty dzień (niedziela, 5 czerwca) będzie zarezerwowany dla okolicy dolnej barku, czyli: patologie splotu ramiennego, w szczególności nerwu pachowego oraz transpozycja ścięgna mięśnia najszerszego grzbietu. Na zakończenie każdego dnia wszyscy członkowie komitetu naukowego będą brali udział w dyskusji, omawiając wykonane operacje.

Podczas transmisji „na żywo” zostaną zaprezentowane wszystkie aspekty postępowania chirurgicznego. W tym roku chcielibyśmy położyć szczególny nacisk na anatomię „na otwarto” i anatomię artroskopową, postępowanie z ubytkami kostnymi w niestabilności barku oraz na wszelkie aspekty rekonstrukcji „na 2 szeregi” stożka rotatorów. Podczas artroplastyki barku chirurdzy będą starali się w sposób szczególny przedstawić dostępy i postępowanie z tkankami miękkimi. Aspekty techniczne użycia poszczególnych typów endoprotez będą omawiane bardziej szczegółowo podczas sesji warsztatowych.

Podobnie jak w roku 2009, stała animacja barku 3D będzie ułatwiała wizualizację anatomii i patologii podczas artroskopii, która jest przecież ograniczonym, dwuwymiarowym obrazem. Celem spotkania w 2011 roku będzie przedstawienie, na podstawie 38 operacji „na żywo”, najnowszych technik operacyjnych oraz pokazanie, w realnym czasie, jak najlepsi chirurdzy barku na świecie radzą sobie z napotkanymi problemami. Jest to najczystszy w swojej formie, najbardziej rzeczywisty pokaz sztuki chirurgicznej. Pomimo okazjonalnie napotkanych trudności, które mogą się pojawiać podczas operacji „na żywo”, będziemy zaczynać i kończyć o czasie. Możliwość śledzenia, jak najlepsi chirurdzy pokonują napotkane problemy jest najbardziej interesująca i stanowi niewątpliwie największy atut tego kongresu. Moderatorzy będą w stałym kontakcie zarówno z obserwatorami, jak i z operującym chirurgiem. Chcemy, abyście byli Państwo aktywnymi uczestnikami dyskusji, co będzie miało niewątpliwy wpływ na korzyści naukowe i miejmy nadzieję, że przyczyni się do uznania tego spotkania za ogromny sukces.

Wij heten u van harte welkom voor de 4^de éditie van de “Annecy Live Shoulder Surgery Advanced Course”.

Het eerste congres dat plaatsvond in 2005 is de grootste Live-Chirurgie bijeenkomst van de wereld geweest. Sindsdien organiseren wij iedere twee jaar een bijeenkomst die wereldwijde interesse wekt. In 2009 zijn er meer dan 1000 mensen, waaronder 800 chirurgen uit 48 landen en 5 continenten bijeen gekomen voor deze gebeurtenis. Wij zijn erg dankbaar en verheugd dat ´s werelds beste chirurgen tijd vrijmaken en hun deskundigheid ter beschikking stellen om deze cursus tot een succes te maken. Hopelijk zullen zowel chirurgen als moderatoren open en intensief samenwerken, zodat de faculteit iedereen diepgaand kan informeren over de meest geavanceerde en bijgewerkte technieken die vandaag de dag mogelijk zijn op het terrein van schouderchirurgie.

De schouderchirurgie gaat verbazingwekkend snel vooruit. In de loop van de laatste twee jaar is onze kennis over de “out of the box” schouderanatomie èn over beschikbare behandelingsopties voor de schouderpathologie drastisch toegenomen. De schouderchirurgen kunnen de pathologie nu beter analyseren en hebben technische geavanceerde behandelmethodes ontwikkeld. De wetenschap vordert zeer snel en het wordt steeds moeilijker om bij te blijven in dit alsmaar veranderende milieu!

Wij organiseren voor dit Live-Chirurgie Congres een directe uitwisseling van chirurgische ideeën en technieken. Tijdens deze vierdaagse bijeenkomst, zullen er 38 live-operaties in onze Internationale Schouderfaculteit worden uitgevoerd. Zoals in 2009 is het concept uitsluitend op live-chirurgie gericht en het leerplan zal op de regionale anatomie en niet op de pathologie worden gebaseerd.

Wij hebben diverse chirurgische procedures geselecteerd om de verschillende pathologie volgens een specifiek schoudergebied te behandelen:

Dag 1, donderdag 2 juni, zal aan de arthroscopische diagnose en behandeling van pathologie van de Postero-Superieure schouder worden gewijd, die omvat: De reparatie van de «SLAP», de labrumreparatie en de posterieure bot- blokkeer procedures, Supra & Infraspinatus cuff reparaties, Suprascapulaire zenuwtoegang en –vrijmaking, en de A-C gewrichtstabilisatie.

Dag 2, vrijdag 3 juni, zal volledig aan de schouderarthroplastie worden gewijd.

Dag 3, zaterdag 4 juni, zal op het arthroscopisch beheer van het anterieure schoudergebied worden gericht, dat omvat: anterieure labrum reparatie, Subscapularis reparatie, evenals de antérieure bot- blokkeer- en de Latarjet procedures.

Dag 4, zondag 5 juni, zal zich op de Inferieure Schouder richten, die omvat: plexus brachialis letsel en specifieker, de Axillaire Zenuwbevrijding en Dorsi Latissimusoverdracht.

Aan het einde van iedere dag komen alle faculteitsleden bijeen voor een definitieve paneldiscussie.

Alle aspecten van het chirurgische beheer voor de schouderpathologie zullen worden getoond. Nochtans, zal er dit jaar meer tijd gewijd worden aan de open- en arthroscopische schouderanatomie en het beheer van botverlies bij schouderinstabiliteit. Evenals de relevante details en opties voor een dubbele rij van rotatorenmanchetfixatie. Het voornaamste doel van de Arthroplastiechirurg is om zich op het beheer van het zachte weefsel en de chirurgische blootstelling A-C gewricht te richten. De prothesen zullen volledig besproken worden tijdens de workshop.

Zoals in 2009, zal er een permanente 3D schouderanimatie plaatsvinden, deze zal ons helpen om de anatomie en pathologie te visualiseren, daar de arthroscopie ons beperkt tot een tweedimensionale visie. Het doel van deze bijeenkomst is om, door middel van 38 live-operaties, de nieuwste technieken te tonen en te ontdekken hoe de chirurgen en hun moderatoren de verschillende situaties en moeilijkheden onder ogen zien.

De mogelijkheid om waar te nemen hoe ’s werelds beste chirurgen en met moeilijkheden geconfronteerd worden, is zeker één van de interessantste en leerzaamste hoogtepunten van deze bijeenkomst. De moderatoren zullen het permanente verband tussen het publiek en de presterende chirurg zijn.

wir heißen Sie herzlich willkommen zur 4. Ausgabe des Live-OP Schulterkurses für Fortgeschrittene in Annecy!

Seit der Auftaktveranstaltung im Jahre 2005 ist dies das weltweit erste und größte Meeting dieser Art, welches ausschließlich aus Live-OPs besteht. Mit großem Aufwand führen wir diesen Kurs mit enormem weltweiten Interesse fort. Im Jahr 2009 haben uns über 1000 Teilnehmer in Annecy besucht, darunter mehr als 800 Orthopäden aus 40 Ländern und 5 Kontinenten. Wir möchten hiermit auch unseren wunderbaren internationalen Referenten danken, welche viele der bekanntesten Schulterchirurgen aus aller Welt einschließt. Die Zeit und Expertise, welche von diesen Kollegen aufgewandt wurde, machen diesen Kurs zu einem Erfolg. Wir hoffen, dass die Fakultät es schaffen wird, mittels Interaktion und Wissensvermittlung, einen Überblick über die neuesten und modernsten Techniken in der Schulterchirurgie zu ermöglichen.

Die Chirurgie der Schulter ist ein relative junges Gebiet und entwickelt sich momentan sehr schnell. In den letzten beiden Jahren ermöglichte ein großer Zuwachs an Wissen vor allem in der extraartikulären Arthroskopie ein besseres Verständnis von Pathologie und Behandlungsmöglichkeiten. Forschung und Wissen vergrößert sich mit einer rasanten Geschwindikeit und es wird zunehmend schwieriger für den Orthopäden, auf dem Laufenden zu bleiben!

Mittels eines Live-OP Kurses ist es uns möglich, die aktuellsten und neuesten Methoden in der Schulterchirurgie zu vermittlen, was einen Schwerpunkt unseres Kurses darstellt. Während des 4-tägigen Kurses werden 38 Live-Operationen von unserer internationalen Referenten durchgeführt. Wie im Jahre 2009 werden wir an dem Konzept festhalten, ausschliesslich Live-Operationen zu zeigen, welche sich an anatomischen Regionen orientiert und nicht an Pathologien. Wir haben Operationen ausgewählt, welche Pathologien haben gemäß einer anatomischen Region.

Der erste Tag (Donnerstag, 2. Juni) ist der postero-superioren Schulter gewidmet: SLAP Repair, posteriore Labrum Refixierung und posteriorer Knochenblock, Supraspinatus und Infraspinatus Refixierung, Release des Nervus Suprascapularis und ACG Stabilisierung.

Am zweiten Tag (Freitag, 3. Juni) wird ausschliesslich Schulterendoprothetik gezeigt.

Am dritten Tag (Samstag, 4.Juni) werden Pathologien der anterioren Schulter gezeigt, wie Bankart Repair, Subscapularis Refixierung, anteriorer Knochenblock und Latarjet Technik.

Am vierten Tag (Sonntag, 5.Juni) konzentrieren wir uns auf die inferiore Schulterregion mit Plexus Brachialis Release, Nervus Axillaris Release und Latissimus Dorsi Tranfer.

Am Ende des jeweiligen Tages versammeln sich alle operierenden Orthopäden zu einer abschliessenden Diskussion

Sämtliche Aspekte der operativen Behandlungsmethoden werden für den jeweiligen Fall gezeigt. In diesem Jahr ist ein Schwerpunkt, die entsprechende Schulteranatomie für offene als auch arthroskopische Fälle darzustellen, die Behandlungsmöglichkeiten für Knochenverlust im Rahmen einer Instabilität zu zeigen, sowie die Feinheiten und Optionen für eine Double Row Rotatorenmanschetten-Refixierung zu vertiefen. Das Ziel des Endoprothetik-Tages ist es, insbesondere den operativen Zugang sowie Weischteilmanagement darzustellen und nicht die sonst übliche Betonung auf die verschiedenen Arten und Typen der Endoprothesen. Diese werden während der Workshops ausführlich erklärt.

Wie beim letzten Kurs im Jahre 2009 nutzen wir virtuelle Schulteranimationen (Live 3D), um Anatomie und Pathologie zu verdeutlichen und die sonst übliche zweidimensionale Ansicht zu erleichtern. Unser Ziel ist es während des Kurses im Juni 2011, mittles 38 Live-OPs die modernsten Techniken in Echtzeit zu zeigen und wie die besten und erfahrensten Orthopäden der Welt mit diesen verschiedenen Situationen und Herausforderungen umgehen. Das ist live, transparente “State Of The Art” Schulterchirurgie. Trotz der gelegentlichen Schwierigkeiten, welche naturgemäß während dieses Kurse auftreten können, sollten alle Fälle pünktlich angefangen und beendet werden. Einer der interessantesten und lehrreichsten Aspekte dieses Kurses ist sicherlich, die bekanntesten und erfahrensten Schulterchirurgen live dabei zu beobachten, wie sie gewisse Probleme lösen und auf Schwierigkeiten reagieren. Die Moderatoren werden dabei als permanente Verbindung zwischen dem Auditorium und den operierenden Orthopäden dienen. Wir wünschen uns, dass Sie als Teilnehmer eine aktive Rolle in diesem dynamischen Prozess spielen, um diesen Kurs wiederum einen Erfolg zu machen!

Bienvenidos a la cuarta edición del Curso Avanzado de Hombro y Cirugías en Directo de Annecy.

Desde el evento inaugural en 2005, este ha sido el primer y más grande congreso del Mundo basado únicamente en cirugías en directo. Hemos continuado organizando este curso bianualmente con gran interés a nivel Mundial. En 2009, más de 1000 personas, incluyendo más de 800 cirujanos de 48 países y 5 continentes se dieron cita en Annecy para el evento. Queremos expresar, a través de estas líneas, nuestro agradecimiento al grupo de expertos internacionales, incluyendo a los mejores cirujanos de hombro de todo el Mundo, que compartieron su tiempo y su experiencia para que este curso fuera un éxito. Esperamos que, en esta nueva edición, y a medida que cirujanos y moderadores enfoquen y profundicen transparentemente en los temas de interés, los expertos os transmitan a todos vosotros un conocimiento profundo de las técnicas más avanzadas y actualizadas de la cirugía de hombro existentes hoy día

La cirugía del hombro está avanzando a una velocidad increíble. En los dos últimos años, se ha producido un aumento espectacular de nuestro conocimiento “Out of the box” (fuera de la articulación) y de la anatomía endoscópica extraarticular del hombro, así como de la patología y de las diferentes opciones válidas de tratamiento disponibles. Los cirujanos de hombro son capaces en la actualidad de analizar mejor la patología del hombro y han desarrollado métodos de tratamiento técnicamente más avanzados para solucionarla. La Ciencia está progresando rápidamente, y cada vez es más difícil que el cirujano se mantenga al margen de este mar revuelto de cambios!

Como curso de cirugía en directo, ofrecemos todo lo posible en tiempo real, con el mayor intercambio posible de ideas quirúrgicas y del estado del arte. Durante los 4 días de duración del evento, nuestros expertos de hombro o Faculty realizarán 38 cirugías en directo. Como en 2009, nos ceñiremos al concepto de Cirugía en directo exclusivamente, y el guión se basará en la anatomía regional y no en la patología. Hemos seleccionado diferentes procedimientos quirúrgicos acordes con cada región específica del hombro que recorrerán las diferentes patologías.

El día uno (Jueves 2 de Junio) se dedicará al diagnóstico y tratamiento artroscópicos del hombro Postero-Superior, incluyendo: reparaciones de SLAP, reparaciones del labrum posterior y procedimientos de tope óseo posterior, reparaciones de supra e infraespinoso, abordaje y descompresión del nervio supraescapular, y estabilización de la articulación acromioclavicular

El día dos (Viernes 3 de Junio) se dedicará plenamente a la Artroplastia

El día 3 (Sábado 4 de Junio) se centrará en el manejo artroscópico de la región Anterior del hombro, incluyendo: Reparación del labrum anterior, reparación del tendón subescapular, así como tope óseo anterior y la técnica de Latarjet.

El día 4 (Domingo 5 de Junio) se centrará en la región Inferior del Hombro, incluyendo: El plexo braquial y más específicamente liberación del nervio axilar y trasferencia del dorsal ancho. Como conclusión de cada día, todos los expertos comparecerán en una discusión final.

Se mostrarán todos los aspectos del manejo quirúrgico de las patologías citadas. Sin embargo, este año se dedicará más tiempo a la anatomía abierta y artroscópica del hombro, al manejo de la pérdida ósea en el contexto de la inestabilidad, y a los detalles y opciones concretas para la reparación en doble hilera del manguito rotador. El objetivo para el cirujano que lleve a cabo cada artroplastia será profundizar en el manejo de las partes blandas y en el abordaje quirúrgico de la articulación, más que enfatizar en el tipo de implante utilizado. Las prótesis se discutirán ampliamente en las sesiones de trabajo.

Como en 2009, un hombro virtual en 3D permanente ayudará en la visualización y discusión de la anatomía y la patología tratadas en cada artroscopia, limitada por la visión en dos dimensiones. El objetivo del curso de 2011 es mostrar, a través de 38 cirugías en directo, las técnicas más actuales, y aprender, en tiempo real, cómo los mejores cirujanos del mundo y los moderadores se enfrentan a las diferentes situaciones nombradas. Eso es el Directo, transparencia en el estado del arte de la cirugía del hombro. A pesar de las dificultades , en ciertas ocasiones, que transcurren en la cirugía en directo, empezaremos y finalizaremos a la hora. La oportunidad de ver cómo los mejores cirujanos de todo el mundo reaccionan y afrontan esas dificultades es ciertamente una de las características más interesantes y plato fuerte de este curso. Los moderadores serán el nexo de unión permanente entre la audiencia y el primer cirujano. Queremos también que forméis parte de esa discusión abierta y de este proceso de aprendizaje dinámico, formando parte del éxito de este evento!

Welcome to the 4th edition of the Annecy Live Surgery Shoulder Advanced Course.

Since the inaugural event in 2005, this has been the world’s first and largest meeting built exclusively on live surgery. We have continued to organize every 2 years this course with enormous world-wide interest. In 2009, over 1000 people, including more than 800 surgeons from 40 countries and 5 continents gathered in Annecy for the event. We extend our gratitude to the wonderful international faculty, including the best shoulder surgeons from around the world, for donating their time and expertise to make this course a success. Hopefully as both surgeons and moderators, interacting and sharing transparently, the expert faculty will provide all of you with an in-depth knowledge of the most advanced and up-to-date techniques available in shoulder surgery today.

Shoulder surgery is advancing at an amazing rate. Thus, over the last two years, there has been a dramatic increase in our knowledge of the “Out of the Box “ endoscopic extra-articular shoulder anatomy, pathologies and the relevant treatment options available. Shoulder surgeons are now able to better analyze shoulder pathologies and have developed more technically advanced methods to address them. Science is progressing very quickly, and it is becoming increasingly difficult for the surgeon to keep pace in this rapidly changing environment!

As a live surgery course, we provide the most real-time, up-to-date exchange of state of the art surgical ideas and techniques, and we continue to organize the course with this in mind. During the 4 day event, our international shoulder faculty will perform 38 live surgeries. As in 2009, we will adhere to the concept of exclusively live surgery, and the curriculum will be based on regional anatomy and not pathology. We have selected surgical procedures to deal with a variety of different pathologies according to a specific shoulder region.

Day one (Thursday June 2nd) will be dedicated to arthroscopic diagnosis and treatment of pathology of the Postero-Superior shoulder, including : SLAP repair, Posterior labral repair and posterior bone block procedures, Supra & Infraspinatus cuff repair, Suprascapular nerve access and release, and AC joint stabilisation.

Day 2 (Friday June 3rd) will be dedicated entirely to Arthroplasty.

Day 3 (Saturday June 4th) will address arthroscopic management of the Anterior Shoulder region, including: Anterior labral repair, Subscapularis repair, as well as the anterior bone block and Latarjet procedures.

Day 4 (Sunday June 5th) will focus on the Inferior Shoulder, including: the Brachial Plexus and, more specifically, Axillary Nerve release and Latissimus Dorsi transfer. At the conclusion of each day, all faculty members will gather for a final panel discussion.

All aspects of surgical management for the encountered shoulder pathologies will be shown. However, this year more time will be dedicated to open and arthroscopic shoulder anatomy, management of bone loss in shoulder instability, and to pertinent details and options for double row rotator cuff fixation. The target for the arthroplasty surgeon will be to focus on soft tissue management and surgical exposure of the joint rather than on the specific types of arthroplasties used. The prostheses will be fully discussed during the workshop sessions.

As in 2009, a permanent live virtual 3D shoulder animation will aide in visualization and discussion of the anatomy and pathology currently treated under arthroscopic, limited, two-dimensional visualization. The aim of the 2011 meeting is to show, through 38 Live Surgeries, the most up-to-date techniques, and to learn, in real time, how the world’s best surgeons and surgeon moderators face the different situations and difficulties encountered. This is live, transparent, state of the art shoulder surgery. Despite the occasional difficulties that arise during live surgery, we will start and finish on time. The opportunity to observe how the best worldwide surgeons react and confront these difficulties is certainly one of the most interesting and instructive highlights of this meeting. The moderators will be the permanent link between the audience and the performing surgeon. We also want you to be an active part of the discussion and this dynamic learning process as you will make this event a success!
